APPOINTMENTS AND CANCELLATIONS
Our philosophy is to provide high quality medical care by being a low volume medical office. We schedule our patients in such a way that they receive maximum care. Each and every limited appointment space is extremely valuable to our patients. As a courtesy to all, we do not overbook patients to compensate for those who forget to honor their appointment.
We understand that our patients will sometimes experience unforeseen circumstances and/or schedule changes. We ask that you call our office immediately once you realize that you are unable to keep an appointment, preferably with at least 48 hour notice. This allows the office staff an ample amount of time to reschedule another patient into your vacated time slot. With your timely notice, our ability to shuffle patients keeps future appointment availability to a maximum and appointment waiting times to a minimum for you and all of our patients.
For such reasons, we have a strict “Late Cancellation and No Show” policy. If you miss your appointment or cancel less than 24 hours before, you will be charged a fee of $25 per occurrence. If you have missed two or three appointments or more consecutive appointments and have not paid your fees, we reserve our right to refuse any further medical care to you. As harsh as it may sound, our policy is necessary to maintain the high professional standards we practice. Missed appointments and late cancellations deprive other patients of critical and valuable time they could spend with their highly sought-after treating nephrologist.
PRESCRIPTION REFILL REQUESTS
Prescriptions and refills for established patients of Florida Kidney Physicians are routinely issued during our regular office hours from Monday to Friday from 8am to 5pm. No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times.
You can make your request on your patient portal or by calling to your nearest location. Refill requests require a minimum notice of 5 business days – no exceptions. We recommend you notify us 2-4 weeks prior to running out of your last refill.
In order to have your medication refilled, you must be up to date on your follow-up visits. If your prescription bottle indicates you have one (1) refill left, you do not need to contact our office. Contact your pharmacy and they will refill it for you. Be sure to give the pharmacy adequate time to fulfill your request. If there are no refills remaining at the pharmacy, you can ask the pharmacist to contact us for refill authorization.
If you do not have any refills left or wish to change pharmacies, please notify us at least 5 business days before your prescription runs out. Routine refills are handled during office hours on the weekdays only. Due to the large volume of refill requests we receive, we send them electronically to the pharmacy within 5 business days of your request. Please note that if there are any questions, this may cause a delay.
REFERRALS
If your insurance is an HMO/managed care plan, and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit.
Please be sure to ask your PCP at least 3 business days before your appointment. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.
Each patient is responsible for his/her referral as mandated by their individual insurance policy.
PLEASE HAVE YOUR MEDICATION BOTTLES IN FRONT OF YOU TO OBTAIN THE INFORMATION BELOW
If your insurance is an HMO/managed care plan, and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit.
– The name and spelling of your medication
– The dosage of your medication and how many times a day you take it
– Your pharmacy name, location, and phone number of choice
– Please check to make sure that one of our doctors prescribed the medication. On occasion, patients will call in for a refill of a medication prescribed by another doctor. We do not refill medications that our doctors didn’t prescribe.
AFTER HOURS URGENT MEDICAL NEEDS
If you have an urgent medical need and need to talk to our doctors after our regular hours, please call your nearest office. Search your location.
A doctor is on-call 24 hours a day. Your call will be returned promptly.
Note: Prescription refill requests are not considered after hours urgent medical needs. Prescriptions will not be refilled outside of normal business hours. Please refer to the section above regarding prescription refill requests.
INSURANCE / PAYMENT / BILLING
At Florida Kidney Physicians, we accept Medicare and most major insurance plans. Please contact your insurance company, or you may call our office to find out if we accept your plan. Search your location.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. Payments can be made at the office or by using our online bill pay through the patient portal. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.
