Brianna Fleming, DNP, AGACNP-BC, APRN
Brianna Fleming is a board-certified Adult-Gerontology Acute Care Nurse Practitioner with a strong clinical background in nephrology, critical care, transplant medicine, and complex medical and surgical care. She brings extensive experience caring for patients with acute and chronic health conditions, with a particular focus on thoughtful clinical assessment, evidence-based treatment, and clear communication with patients and their families.
Brianna earned her Doctor of Nursing Practice in the Adult-Gerontology Acute Care track from Jacksonville University’s Keigwin School of Nursing, graduating with a 4.0 GPA. She is licensed as an Advanced Practice Registered Nurse in the state of Florida and is nationally certified as an Adult-Gerontology Acute Care Nurse Practitioner through the American Nurses Credentialing Center.
Her clinical background has prepared her to care for patients with a wide range of kidney-related conditions, including acute kidney injury, chronic kidney disease, hypertension, electrolyte and acid-base disorders, and complications associated with dialysis. Her nephrology experience includes outpatient care, hemodialysis management, and inpatient nephrology coverage, with an emphasis on evaluating volume status, blood pressure control, vascular access, anemia, mineral and bone health, laboratory trends, and individualized treatment plans.
Before becoming a nurse practitioner, Brianna developed a broad foundation in high-acuity nursing through roles in cardiovascular intensive care, transplant medicine, medical-surgical care, and perioperative nursing. As a CVICU nurse, she cared for critically ill cardiothoracic and vascular surgery patients, managed complex hemodynamics, supported patients requiring advanced circulatory devices, and administered high-risk medications. Her earlier experience at Mayo Clinic Hospital included caring for liver, kidney, pancreas, and lung transplant candidates and recipients, giving her valuable insight into the complex needs of patients with advanced and chronic disease.
During her advanced practice training, Brianna completed more than 750 hours of intensive clinical experience across medical, surgical, cardiovascular, and neurological intensive care settings. This training strengthened her skills in diagnostic reasoning, acute stabilization, interdisciplinary collaboration, and the management of medically complex patients.
Brianna is also committed to quality improvement and evidence-based practice. Her Doctor of Nursing Practice project evaluated a nurse-led postoperative follow-up program and demonstrated measurable improvements in patient communication and satisfaction. She has also contributed to scholarly work involving complex metabolic and renal conditions, including a clinical review focused on hyperosmolar hyperglycemic state in a young adult with type 1 diabetes and nephrotic syndrome.
At Florida Kidney Physicians, Brianna is dedicated to helping patients better understand their kidney health and feel supported throughout their care. She values building trusting relationships, listening carefully to each patient's concerns, and working collaboratively with physicians and the broader healthcare team to create individualized care plans.
